Four places, mapped independently.
Researchers have spent two decades studying a small number of regions where an unusual share of the population reaches 100. None of them share a language, a government, or a coastline. Here is what the research on each actually shows.

How researchers found the pattern.
The pattern began with a single Italian province. In 2000, the demographers Michel Poulain and Giovanni Mario Pes were mapping unusually high concentrations of the very old across Sardinia when the numbers clustered hardest in one place: the mountainous Nuoro province, and within it, the Ogliastra region. A 1999 survey had already found the area held roughly 13 centenarians per 100,000 people, a notably high concentration.
A 2004 follow-up narrowed the finding further. Longevity was concentrated specifically in Sardinia's mountain villages, where men born locally lived measurably longer than men elsewhere on the same island, though the researchers involved were careful to note that the reasons behind it were not yet established.
Starting in 2005, researchers extended the same mapping method to three more regions that showed a similar pattern independently: Okinawa in Japan, the Nicoya Peninsula in Costa Rica, and the island of Ikaria in Greece. None of the four were compared against each other at the outset. Each was documented on its own terms, using the birth and census records specific to that country.
What follows is a reading of each place in turn, and of the pattern researchers describe when they look at all four together.

The Mediterranean two, Sardinia and Ikaria.
Sardinia and Ikaria share a geography before they share anything else. Both are mountainous, both isolated by terrain rather than distance, and both have populations that spent generations walking, farming, and herding across slopes rather than flat ground, a pattern researchers studying physical activity in the region have measured directly, using wearable accelerometers on residents in their eighties.
Ikaria's elderly population has been studied specifically for functional ability rather than survival alone. Researchers documented comparatively high muscle strength and functional independence among residents over 90, despite common age-related health risk factors observed in the same population.
Both regions have also been studied for something less physical. Researchers comparing Sardinia and Ikaria to national averages in their respective countries found a higher share of long-lived residents were married or living with a partner rather than living alone, and a higher reported sense of optimism in both places than in the rest of their countries.
Diet researchers who have studied both regions describe a pattern built around legumes, olive oil, and whole foods grown locally, though the same researchers frame this as an observed association rather than a proven cause. A study directly comparing Sardinia's Ogliastra region with Nicoya, Costa Rica noted that further research would be needed to establish whether diet itself explains the pattern or simply accompanies it.
Okinawa and Nicoya share almost nothing geographically with Sardinia and Ikaria, and were documented separately.
The terrain

Neither region is flat. Researchers studying both have pointed to the terrain itself as a source of daily, unavoidable physical activity.

The Pacific two, Okinawa and Nicoya.
Okinawa's centenarian concentration has been documented in Japan's own national census data for decades, historically one of the highest rates recorded anywhere in the country. Traditional diet in the region has been studied for its reliance on soy-based foods, and daily movement practices, including tai chi, have been documented as a common feature of older residents' routines.
Nicoya's pattern looks different on paper. Researchers there have documented a diet built around beans and corn tortillas, alongside a notably high rate of residents who continue physical, often agricultural, work well into their eighties and nineties, rather than retiring from labor entirely.
Nicoya has also drawn attention from researchers studying something harder to measure: a reported sense of life purpose that residents describe in interviews, sometimes tied to community involvement, sometimes to religious practice. Anthropologists studying the region treat this as a genuine, documented pattern in resident interviews, while noting it is harder to quantify than a census count.
As with Sardinia and Ikaria, researchers studying Okinawa and Nicoya describe overlapping features rather than a single shared cause. Diet, daily movement, and social integration all appear together in the data, without any one factor isolated as the explanation.

What the research doesn't settle.
The word researchers return to most often across all four regions is association, not cause. A study directly comparing the oldest residents of Ogliastra, Sardinia and Nicoya, Costa Rica noted that establishing a cause-and-effect relationship between diet and a longer lifespan would require further prospective research, a caution that applies as much to movement and social patterns as it does to food.
Verification is the other recurring caveat. Birth registration was inconsistent or absent in parts of the world a century ago, and researchers studying regions with unusually high reported rates of extreme age have raised questions about how many of those ages can be confirmed against an original record, a point covered in more depth in a companion piece on how many people live past 100.
Additional regions, including a religious community in Loma Linda, California, have since been studied for similar patterns, which suggests the four original regions are not a closed list so much as the first four places researchers happened to document closely enough to publish.

Reading the map forward.
Four regions studied independently, using four different countries' record-keeping systems, converging on a similar set of observations, is itself the finding researchers keep returning to. It is a stronger form of evidence than a single study from a single place would be, precisely because none of the four teams set out to confirm what another had already found.
What it does not do is identify one ingredient, one habit, or one place as the explanation. The research describes overlapping patterns across geography, diet, movement, and social structure, not a formula that can be extracted from any single region and applied elsewhere.
That is the more accurate way to read four places on a map: not as a destination, but as four independently documented data points in a much larger and still unfolding body of longevity research.
